MY CARE feedback survey – Privacy notice
Summary of Initiative
At the Royal Devon, our electronic patient record (EPR), Epic, is a key enabler for our work to improve care for patients by using digital innovations. It allows us to join up care for our patients, and via MY CARE give patients secure access to their clinical team and hospital information wherever they are. We are working hard to get more out of MY CARE and are exploring ways it can give patients more control over their care, improve their experience and help us be more productive.
We are carrying out a survey of MY CARE users to:
- Understand user satisfaction with MY CARE – including new developments
- Understand what users think are the key benefits and areas of improvement to inform our development plans
- Understand the specific views of proxy users and differences across different geographies and other groupings
What personal data we collect
You can provide personal experiences of using MY CARE but are asked (as far as possible) not to share personal identifiable information.
The survey asks you to state your race or ethnicity, which is a special category of personal data. This is not mandatory, and you are under no obligation to provide this information.
If you are happy for us to get in touch with you to discuss your feedback in more detail and to support communications for the One Devon EPR programme, you can provide your name, email address and phone number. You do not need to provide this to complete the survey.
The personal information you share in this survey will be held for a year before being deleted.
The One Devon EPR programme is a partnership of the Royal Devon, Torbay and South Devon NHS Foundation Trust and University Hospitals Plymouth NHS Trust. Together we are working to implement a single EPR across all three Devon acute Trusts.
How we use your data (purposes)
The data you submit will be used to provide broad analyses of user feedback on MY CARE.
Anonymised data will be published as part of the final survey report and used to inform future development of MY CARE.
Your anonymised comments may be published verbatim in internal and external communications created by the One Devon EPR programme, which may include on websites, social media and other publications.
We are gathering case studies to get a more in-depth understanding of the benefits and difficulties patients experience with MY CARE. We will only use your name, email address and telephone number to contact you about your feedback in this survey for a potential case study. By providing this information you consent to us sharing your details with the One Devon EPR programme.
