What is Personalised Stratified Follow-Up and Remote Monitoring?
Personalised Stratified Follow-Up means having the right type of follow-up for your type of cancer including getting support and information from your healthcare team in a different way, for example, instead of having standard face to face appointments we may ask you to contact your cancer team when you need support or notice new changes as described in your End of Treatment Summary.
Types of follow-up care are:
- Supported self-management – this is when you are given information to help you self-manage and you can contact your cancer team if you need to or notice any changes or new symptoms
- Remote Monitoring – this is where we ask you to have some regular blood tests, scans, or both, and either phone or write to you with the results. You will still be able to phone us for information and support if you notice any changes or new symptoms
- Face to face – this is where you will continue visiting the hospital to see your cancer team face to face at agreed times.
It is important to remember that you will be advised of the best follow-up option for your cancer type. You will be able to contact your Clinical Nurse Specialist or Cancer Support worker for support.
