Community Nursing

The Community Nursing Service is there to provide specialist nursing care in your home environment when you are unable to access this in an alternative setting. This may be short-term due to a specific treatment plan or if your health has deteriorated.

Your community nursing service will help and support you to maintain maximum independence with an aim to self-manage where appropriate and access appropriate services as soon as you are able. We believe patients are entitled to high-quality care, while respecting confidentiality, dignity, multicultural and religious beliefs.

We believe in working in close partnership with family, carers and other health and social care providers to ensure a seamless patient-centred service. We are knowledgeable and highly-skilled professionals providing quality nursing care to patients in our community.
