Care Homes Team
The Northern Care Home team have representation in each Community Health and Social Care Team and work alongside local providers to support and promote effective interventions for our patients living in care homes. The team’s objectives are to improve the care delivered to patients, deliver an equal service to all care providers in our area, to develop open and honest positive relationships with providers and other professionals and to foster positive attitudes toward safeguarding.
What services do we offer?
The Care Homes Team provides support and structured education/training to care homes (nursing and residential), domiciliary care providers and supported/assisted living providers within North Devon.
What locations do we cover?
Our service is split across North Devon and stretches all the way to Bude and Holsworthy, serving a wide population across our community settings. We often refer to individual areas as 'clusters', which helps us provide the right care, at the right time, in the right setting.
We currently have the following clusters:
- Ilfracombe and South Molton
- Barnstaple
- Torridge (Bideford, Northam, Torrington, and Hartland)
- Holsworthy, Bude and Surrounding Villages
