Health and Social Care team

The Adult Health and Social Care team helps to ensure that patients within the community receive the right care at the right time at the right place and that it is delivered by the right person. We focus on supporting people with long-term conditions and/or complex needs to stay well and independent in the community. The teams are made up of a range of health and social care professionals including – therapists, nursing staff, community matrons, social care professionals and representatives from the voluntary sector.

What services do we offer?

The services provided by each cluster vary but will include some or all of the services described below.

  • Community Rehabilitation Team
  • Lymphodema Service
  • Respiratory Outreach Service and Pulmonary Rehabilitation Service
  • Community Nursing Team
  • Pathfinder Service

We provide assessment and treatment for adults over the age of 18. We are able to offer a range of support, which may include:

  • Assessment of ability to remain independently at home.
  • Support and services to remain at home, where appropriate
  • Support transition to home from hospital or other care setting.
  • Provision of support and advice to carers/families.
  • Holistic assessment of daily living skills, which may lead to
  • Assessments and treatments
  • Strength and balance classes
  • Cardiac rehabilitation classes
  • Provision of equipment/adaptations
  • Signposting to other appropriate services
  • Day treatments
  • End of life care
  • Financial assessment and advice

The core working hours of our teams range from 8.00am to 5.00pm, Monday to Friday (except Bank Holidays). Outside of normal working hours, we also provide urgent care nursing and rapid response services which are accessed via 111 or emergency duty team for social care on 0345 155 1007.

What locations do we cover?

Eastern services

Our service is split across East and Mid Devon and Exeter, serving a wide population across our community settings. We often refer to individual areas as 'clusters', which helps us provide the right care, at the right time, in the right setting.

We currently have the following clusters:

  • Crediton, Moretonhampstead and Okehampton
  • Honiton, Ottery St Mary and Cranbrook
  • Exmouth, Woodbury, Budleigh
  • Exeter – South and West
  • Exeter – Central and East
  • Tiverton and Cullompton
  • Sidmouth, Seaton and Axminster

Northern services

Our service is split across North Devon and stretches all the way to Bude and Holsworthy, serving a wide population across our community settings. We often refer to individual areas as 'clusters', which helps us provide the right care, at the right time, in the right setting.

We currently have the following clusters:

  • Ilfracombe and South Molton 
  • Barnstaple 
  • Torridge (Bideford, Northam, Torrington, and Hartland)
  • Holsworthy, Bude and Surrounding Villages

How can I be referred?

Referrals to our services can be made via your GP or from other health and social care professionals for health provision or via Care Direct Plus for social care needs. A member of the team will contact you to discuss your needs and may arrange to visit you in your home, or other appropriate setting, depending on your situation.
