Community Children's Nursing Team - North
A community children’s nurse (CCN) is a registered nurse with special qualifications for working with children. A CCN works in partnership with parents and other health care professionals to ensure that the health care needs of sick children are met.
We provide home-based nursing care for children between 0-18 years old. Children can also be seen in a variety of convenient locations; either schools, hospitals or local clinics

Information for healthcare professionals
Referrals can come from both hospital and community based healthcare professionals. Children / young people may be followed up by a CCN following admission to the children’s ward, Day Surgery Unit, Special Care Unit or from Emergency Department.
