Renal supportive care

Our service aims to support patients and their families with progressive chronic kidney disease (CKD) to live as well as possible by better managing their symptoms. This includes advanced care planning and end of life care for those on a supportive care pathway, but also those previously on a dialysis pathway choosing to withdraw from renal replacement therapy.

The renal supportive care team are based within the renal unit and work closely alongside the trust palliative care team for inpatients and our community renal and non renal colleagues for outpatients to support holistic patient focussed care and signposting to relevant other services.

Our office base is in the renal unit on the Wonford Site. We are happy to discuss any supportive care issues with regard to renal patients but to formally refer patients they need to be known to a consultant nephrologist. Our service is available Monday to Friday, 830-430.

Our team consists of: consultants, doctors, dietitians, clinical nurse specialists, renal nurses, assistant practitioners and healthcare assistants. Supported by an administration team and renal technicians.
