North Devon Integrated Diabetes Service

The North Devon Integrated Diabetes Service project is working to develop person-centred diabetes care integrated across primary and secondary care and delivered consistently across North Devon.

Where clinically required this care will be in hospital, whilst all other diabetes care will be delivered nearer to home enabling a more holistic service where people with diabetes and their families are empowered, manage their health more effectively and reduce the likelihood of further complications by receiving the advice, support and care that they need when they need it.

Who we are

Our project team includes representation from diabetes specialists, GPs, patients, Diabetes UK, commissioners, mental health, service managers, dietitians, podiatrists, community nursing, practice managers, clinical pharmacists, microbiologists, tissue viability, domiciliary care providers.

We work with people with diabetes, asking them what would help them manage their condition, and involving primary care services, secondary care services, and commissioners with our research. 

We work together to help improve our diabetes services and support excellent care provision across North Devon, focusing on key issues that we identified with our research.
