One Northern Devon’s High Flow team win at Pride of DPT awards

One Northern Devon’s High Flow team were winners in the Partnership award category at the Pride of DPT Awards Programme ceremony on Tuesday 3 December at Exeter Racecourse.
The award recognises outstanding collaboration between DPT teams and colleagues from partner organisations, that significantly enhance the care and support available for those in our communities. This award celebrates initiatives where working together has led to innovative solutions, improved patient outcomes, or a more seamless experience for those in care by working beyond boundaries.
Case workers work with individuals on what matters to them, setting goals and supporting them to reach these, whether that’s joining a community group to reduce isolation or supporting people with addiction and housing issues. So far this year, 46 people have been supported by the team, which has reduced emergency admissions in these individuals by 79% and many of them have been supported with their mental health needs.
One individual supported by the team said: “When we first met, I was really scared because I thought you wouldn’t like me, but without you I would probably be dead. I don’t want to talk about you finishing your work with me, but I know we have to. Your help has had a massive impact.”
One Northern Devon’s High Flow team are a partnership team from the Royal Devon, DPT and Encompass South West. High Flow is a personalised and holistic support programme that sees flow caseworkers work one-to-one with people who are frequent users of ED, and who often have complex needs, a history of trauma and multiple disadvantages.
