Urgent Care Nursing Team
The Urgent Care Nursing Team provides out-of-hours urgent care visits to assist individuals to improve, maintain or recover from their health condition and to provide support and care to those with life-limiting illnesses. The service is aimed primarily at those who are housebound and aged 18 and above.

The service aims to:
- Prevent avoidable admissions to hospital by offering support and assessments in the community and facilitate early discharge from hospital.
- Provide nursing care and holistic support to patients and their families at end of life, this includes supporting with relief of symptoms, physical and emotional nursing care.
- Provide treatment and intervention for clinical conditions which require treatment at home.
- Provide long term condition management and sustainability whilst at home.
- Provide supportive home-based interventions through recuperative stages of care following surgical and medical intervention in hospital.
- Support safe and effective medicines management.
- Provide education, support and signposting for both informal and formal carers.
What locations do we cover?
The urgent care Nursing Team cover all the GP surgeries in the Northern Locality. The catchment area is a 1000 square miles
